Abstract:

Abstract Objectives Identify facilitators of the practice of Responsive Feeding/Parenting (RF/P) among parents of children under 3 years of age in the Central Region of Ghana. Methods Qualitative data was collected through six audiotaped focus group discussions with mothers (n = 27) and fathers (n = 18) living in the Central Region of Ghana who a) were 18 years of age or older; b) had a child under 3 years of age; c) were involved in feeding the child. Transcripts were read and coded independently by three authors, consensus was reached about emerging domains and themes, and a final codebook was developed. Results Three factors were identified as direct facilitators of the practice of RF/P: support, health care providers infant and young child feeding (IYCF) education delivery, and IYCF practice. Family, friends and the community gave parents informational, instrumental and emotional support including education/counseling on childcaring practices, doing chores for parents, giving parents the time off to respond to their children's needs. This support enabled parents to learn and adopt some RF/P behaviours. Health care providers delivered education on IYCF, food safety and, in some instances, provided some RF/P counselling/guidance. This then enabled parents to identify the nutritional and the psychosocial needs of their children and to recognize they should attend to these needs using RF/P knowledge and practice. Non-responsive feeding/parenting practices also emerged within IYCF practices demonstrating the need for more high-quality RF/P education. Conclusions Health care providers as well as family/friends facilitate parent's IYCF practices, which in turn influences their RF/P practices. Strengthening IYCF knowledge delivery through enhanced antenatal and postnatal counselling targeting parents as well as family/friends can promote RF/P knowledge and practices among Ghanaian parents. Abstract:

There is much to admire about community schools, which provide students and their families with wrap-around services focusing on mental health, nutrition, college counseling, and other needs. However, says PDK’s Joshua Starr, the community schools movement needs to address a couple of problems that could undermine its work over the long term — one problem has to do with its use of resources and the other with its use of data.

Abstract:

This article investigates National Defense Education Act and National Defense Education Act-related calls in the late 1950s for the training of guidance counselors, an emergent profession that was to play an instrumental role in both the measuring and placement of students in schools by “intelligence” or academic “ability”. In analyzing this mid-century push for more guidance counseling in schools, this article will first explore a foundational argument for the fairness of intelligence testing made by Educational Testing Service psychometrician William Turnbull in 1951, and then later taken up and employed by other National Defense Education Act-era advocates of testing and grouping. Secondly, this analysis will proceed to National Defense Education Act expert testimony, examining here assertions of the necessity of guidance counseling in schools, and an emergent and shared vision articulating the role guidance counseling was supposed to play in school life. A pattern or structure to this vision emerges here. According to its advocates, guidance counseling would not only inform the self-understanding of the measured individual, but it would also work to condition the ideology of individual intelligence across numerous layers of social life around the student: through peer group, through teachers and school administrators, and finally through home, family, and the wider community.

Abstract:

This study aims to analyze the use of learning resources in the field of counseling guidance. This study uses a qualitative research method based on library research (library research). Collecting data using documentation techniques derived from library sources. Data analysis uses content analysis techniques where each source or literature used is adjusted and the context's suitability is seen with other literature from books, journals or other research. At present the quality of learning and community education is strongly influenced by the conditions of existing learning resources, and this cannot be denied because if the conditions and availability of learning resources are adequate, the learning process can run well. This indirectly shows how important it is to use learning resources in a learning process. In the realm of guidance and counseling, learning resources are closely related and are often used in various implementation of guidance and counseling services, both in the formal and non-formal world of education, in this case the associated learning resources can be in the form of religion or instruments used in the successful implementation of guidance and counseling service.

Abstract:

Currently, all members of the educational community have a relevant role in the development of professional competences on the sustainability of students. This is supported by the findings obtained by different research studies carried out in different countries and which show the need to characterize and evaluate the practice of EGS as key agents in the training and counseling of teachers on sustainability topics, which, in turn, is the goal of this study. To address this, we have conducted an exploratory study based on the case study of EGS members in the metropolitan area of Granada (Spain) that has helped us to profile the professional practice of its members in the development of professional competences in Education for Sustainable Development (ESD). Answers to a questionnaire provided by 43 teachers who are linked to the EGS have been investigated in addition to an interview to six formal members of the EGS. Results evidence that transmit some values over others correlates positively with the type of competences for sustainability that they promote. Therefore, this shows the interrelation of these competencies with the teaching-learning process and, consequently, with the promotion of an education for sustainability. We conclude that counseling on participatory methodologies, working on values, competences, and curricular sustainability, as well as the cross-curricular nature of the content that is imparted, are the most effective ways from which these teacher guidance services can promote the development of professional competences linked to sustainability in schools.

Abstract:

The role and training of Queensland guidance officers appears to be set toward the practice end of the scientist-practitioner continuum. Yet data are presented which demonstrate that the expectations placed on many guidance officers are relatively sophisticated and specialised. Particularly in the areas of personal crisis counselling, family support and community-school development, today's guidance officers working in Queensland schools are required to demonstrate considerable specialisation. The profession seems to be facing a choice, attempt to develop into a group of true educational specialists with all the required training, supervision and in-service education or move to become a group of expert general practitioners. Some implications of these two options for the profession are explored.

Abstract:

Parenting plays a very important role in building children’s character and personality, thus it requires the provision of good education and care (nurture) from parents in order to generate children with good character or moral.Parents, especially mothers, therefore, are demanded to possess good parenting skills. But the reality shows that most mothers in Bandung have poor parentingskillsa. It is shown by the large number of parents who still hold the authoritarian type of parenting, which amounts to 86.7%. This phenomenon ofpood parenting skills of mothers in the city of Bandung underlies the need for the arrangement of effective guidance model to improve the parenting skills of parents, especially mothers as the holder of an important role in the education of children in family. For if these parents are allowed to adopt the authoritarian type of parenting, then the character and personality of the children they bring up will be unfavorable, whereas moral or noble character is an important foundation in the establishment of a secure, serene, and peaceful society. Based on such phenomenon, the researcher tried to design a program of guidance supported with a group approach as an effort to enhance the parenting skills of mothers. The program is tested on mothers participating BKB (Bina Keluarga Balita) in Bandung. After going through a series of trials, the result obtained shows that a group guidance held in eight sessions, with the subject of: (1) Monitoring children's behavior, (2) Having effective communication with children, (3) Setting rules and limits, (4) Enforcing rules without collisions, (5) Stimulating the task of childen development, (6) Being a good listener, (7) Applying Love Languages, and (8) Complimenting children, proved to effectively improve the parenting skills of mothers. It is characterized by theshift of parenting type, from authoritarian to authoritative. In conclusion the counseling program with the group approach can be an alternative model of guidance that can be used to enhance the parenting skills of mothers in a variety of settings in the community; in Majlis taklim, it is one of the alternative methods of da’wa, in BKB (Bina Keluarga Balita) it is a model of community counseling, in early childhood education concerning the co-parenting program, it is an education model for parents.

Abstract:

The public should be aware of the sudden change of life style of some people that makes their life completely different from their previous ones. All of us must be ready to face any circ*mstances from this manifestation in the future. The rich, wealthy and famous should be made aware that that type of life style is not wrong, but they should also be taught how to follow a normal way of life. Proper guidance and counselling on how to live normally needs to be provided with especially designed lectures and classes. Bringing these “students” to remote villages where the poor and the disadvantaged live may help them to understand the hardships of life around them. Having a short intern period within such a community will help them further understand the real life of the majority in their country. Furthermore, if they are always taught in an environment of luxury and are never faced with a normal education system and style of life, a major problem arises: if, in the future, our country is led by such graduates, who never went through our normal education system or lived a normal style of life, how can they understand and lead the majority in our country? This paper discusses this phenomenon.

Abstract:

We present the first results of an empirical study, whose purpose is to analyze the level of satisfaction and difficulties encountered by guidance teachers from Cantabria. The aim of this the study is to analyze the new counseling model carried out in Cantabria from the 2005-6 school year, which deviates from the LOGSE model but does not completely abandon it. Guidance teachers are a key element in the counseling model of this community. This new counseling model will be analyzed as a result of the opinions expressed by them and their proposals to improve it.From the methodological point of view, a qualitative approach is adopted to triangulate the information, carried out through questionnaires, interviews and discussion groups. Twelve centers from Cantabria and all the guidance teacher groups of the Community have collaborated in the project. 62 primary and secondary guidance teachers participated in the whole study. Questionnaires, which are analyzed in other studies, have also been applied to primary and secondary school principals and tutors. The lack of time to perform all the functions attributed to the counselor appears as a key difficulty, followed by the complexity of the students' needs and the excessive dependence on guidance teachers.

Abstract:

This study investigated the challenges and counselling needs of sickle cell patients attending the University of Ilorin Teaching Hospital (UITH) Kwara State, Nigeria. Purposive sampling was used to select 120 literate regular and non-regular patients of the hospital. A researcher developed instrument titled “Challenges and Counselling Needs of Sickle Cell PatientsQuestionnaire” (CCNSPQ) was used to elicit information from these participants. Five research questions raised in the study were answered using frequency counts, means and ranking and Analysis of Variance was used to test the hypotheses formulated at the .05 level of confidence. Thefindings showed that excessive bone, abdominal and chest pains are the major challenges confronting the respondents while occasional embarrassment due to unexpected sickness accompanying sickle cell disorder and stigma are the most serious counselling needs. There was nosignificant difference in the challenges of sickle cell patients based on age but the respondents had significantly different counselling needs. Therefore, it is recommended that guidance and public health education be put in place to reduce the stigmatisation of sickle cell patients. In addition, wellequipped clinics should be provided to facilitate early treatment. Genetic Counselling should also be provided both in the school and the community to prevent future occurrence of sickle cell disorder. Keywords • Sickle cell disorder • Genetic Counselling • Higher Education

Abstract:

Background and context: Since 1982, cancer has been the leading cause of death in Taiwan, claiming more than 40,000 lives each year. This not only caused huge medical expenses, but also affected the quality of life of patients and their families. However, many cancer survivors and their caregivers do not fully understand lifestyle advice, including nutrition and dietary behaviors, to lower the progression of the disease. Due to cancer and painstaking treatment, cancer patients often suffer from inadequate calories intake and serious body weight (BW) loss, which is highly related to malnutrition or cancer cachexia. After they leave the hospital, they still need nutritional guidance; therefore, the importance of providing nutrition services in the community should be emphasized. Aim: To help cancer survivors achieve better nutritional status by teaching them how to have adequate calories intakes and maintain BW that they're supposed to have better quality of life. Strategy/Tactics: (1) Cancer survivors with nutritional needs were referred from 66 cooperative cancer resources centers of hospitals nationwide. (2) Dietitians assess their nutritional conditions and provide nutritional guidance. (3) Deliver free nutritional supplements to the cancer survivors who are financially disadvantaged or have dysphagia problems. Program/Policy process: The registered dietitians conducted nutritional education through nutrition counseling and guidance. For those who are financially disadvantaged or have dysphagia problems, the 24-hour dietary recall and PG-SGA scale were used to assess the survivors' nutritional status, including BW and calories intake, then free nutritional supplements according to their needs and a regular follow-up to collect their BW and nutritional information changes after our interventions were done. Outcomes: From 2016 to 2017, a total of 434 of cancer survivors who have financial difficulties or dysphagia problems accepted the free nutritional supplements and nutritional guidance services. 178 survivors completed follow-up and collected nutritional information. 40.4% of them are head and neck cancers, 38.2% are digestive system-related cancers that were in poor eating conditions. After our interventions, 70.2% of these survivors can maintain or increase their BW with average BW 57.9 ± 12.8 kg; and 77.0% can maintain or increase the calories intake, which average increased from 1798 ± 252.5 kcal/day to 1541.6 ± 347.9 kcal/day. What was learned: We can effectively help cancer survivors achieve adequate calories intakes and maintain BW to prevent the occurrence of malnutrition by providing the services of nutritional supplements and nutritional guidance.

Abstract:

BackgroundAPOL1 variants contribute to the markedly higher incidence of ESKD in Blacks compared with Whites. Genetic testing for these variants in patients with African ancestry who have nephropathy is uncommon, and no specific treatment or management protocol for APOL1-associated nephropathy currently exists.MethodsA multidisciplinary, racially diverse group of 14 experts and patient advocates participated in a Delphi consensus process to establish practical guidance for clinicians caring for patients who may have APOL1-associated nephropathy. Consensus group members took part in three anonymous voting rounds to develop consensus statements relating to the following: (1) counseling, genotyping, and diagnosis; (2) disease awareness and education; and (3) a vision for management of APOL1-associated nephropathy in a future when treatment is available. A systematic literature search of the MEDLINE and Embase databases was conducted to identify relevant evidence published from January 1, 2009 to July 14, 2020.ResultsThe consensus group agreed on 55 consensus statements covering such topics as demographic and clinical factors that suggest a patient has APOL1-associated nephropathy, as well as key considerations for counseling, testing, and diagnosis in current clinical practice. They achieved consensus on the need to increase awareness among key stakeholders of racial health disparities in kidney disease and of APOL1-associated nephropathy and on features of a successful education program to raise awareness among the patient community. The group also highlighted the unmet need for a specific treatment and agreed on best practice for management of these patients should a treatment become available.ConclusionsA multidisciplinary group of experts and patient advocates defined consensus-based guidance on the care of patients who may have APOL1-associated nephropathy.

Abstract:

Fathers play a critical role in children’s development; similarly, fatherhood positively affects men’s health. Among the larger population of fathers relatively little is known about the parenting knowledge of urban, African American fathers. Focusing on urban, African American fathers, the objectives of this study were to (1) understand the primary sources from which fathers learn about parenting, (2) determine where and how fathers prefer to receive future parenting education, and (3) explore the information perceived as most valuable to fathers and how this compares with the recommended anticipatory guidance ( Bright Futures-based) delivered during well visits. Five focus groups, with a total of 21 participants, were conducted with urban fathers at a community-based organization. Study eligibility included being more than18 years old, English speaking, and having at least one child 0 to 5 years old. During the focus groups, fathers were asked where they received parenting information, how and where they preferred to receive parenting information, and what they thought about Bright Futures parenting guidelines. Fathers most commonly described receiving parenting information from their own relatives rather than from their child’s health care provider. Most fathers preferred to learn parenting from a person rather than a technology-based source and expressed interest in learning more about parenting at community-based locations. Although fathers viewed health care providers’ role as primarily teaching about physical health, they valued Bright Futures anticipatory guidance about parenting. Fathers valued learning about child rearing, health, and development. Augmenting physician counseling about Bright Futures with community-based parenting education may be beneficial for fathers.

Abstract:

Obesity has become one of this country's most significant nutritional diseases. The prevalence of childhood and adolescent obesity has steadily increased over the past 20 years. The risk of developing health problems increases as the obese child becomes an obese adolescent and adult. The 1997 Youth Risk Behavior Survey reports adolescents participate in behaviors that put them at risk for obesity. The recognition of these unhealthy behaviors among our youth has led to the need for early intervention. The Coodinated School Health Program is a mechanism to address adolescent obesity at the school-age level. This program includes an organized set of policies, procedures, and activities intended to protect and promote the health and well-being of students and staff. The eight components of a Coordinated School Health Program (CSHP) include school health services, a healthy school environment, comprehensive school health education, counseling and guidance, physical education, food service, worksite health promotion, and the integration of school and community. This paper describes integration of prevention and treatment strategies for adolescent obesity for each of the eight components of a CSHP.

Teenage pregnancy outcomes have become an increasing concern in the United States. Education and support of pregnant teens are critical factors that may determine good or poor pregnancy outcomes. Poor outcomes may include low birth weight, developmental delays, and poor academic performance. Although the number of teenagers experiencing pregnancy and parenting has declined in the U.S., school-based health clinics can be used to provide support and guidance designed to avoid the negative outcomes associated with teenage pregnancy and parenting. By having school-based health clinics, nurse practitioners and school nurses can provide much needed services to pregnant and parenting teens. These services should include educational support, counseling, and community resources. This inquiry provides a metasynthesis of the literature and will review, examine, and summarize the literature relating to the effect of school-based clinics on teenage pregnancy and parenting outcomes.

Abstract:

Reproductive health and adolescent understanding of the dangers of free sex and sexually transmitted diseases in Indonesia is still low, this can be seen from the many cases of pregnancy outside of marriage, dating violence and abortion with high-risk drugs. Adolescent's understanding of reproductive health is a provision for adolescents to behave in a healthy and responsible manner. Not all adolescents have received sufficient and correct information about reproductive health. Street children are a group of children who are on the side of the road or on the road, who carry out activities in the hope of earning money for life. Knowledge and attitudes towards personal hygiene of reproductive organs are also poor in terms of research results. Therefore they need to be given counseling, guidance and support, so that healthy growth and development can occur so that one day these teenagers will become healthy adults physically, spiritually and socially through the formation of health cadres. This community service is carried out on street teenagers in the Community Caring for Children (KOPA) Medan Maimun. Medan city. The methods used in this activity are: 1. Health education with material on adolescent reproductive health, the dangers of free sex behavior and sexually transmitted diseases; 2. Cadre training; 3. Assistance of cadres in conducting counseling to peers (peer group). 4. Evaluation of the success of the program: post-test for outreach cadres and youth participants. The data analysis used was univariate. The number of respondents 25 people. The results showed that the majority of the Padang ethnic group (76%), junior high school education (54%), father and mother still (father 80% and mother 92%), education of 80% high school parents. The results of the questionnaire on knowledge were still there (44%) street children had not answered questions about the symptoms of HIV/AIDS. Symptoms of HIV/AIDS include fever, night sweats, continuous diarrhea, fatigue, and persistent coughing and weight loss. They also have not all answered correctly on the question of gangrene is a type of sexually transmitted infection (24%), mode of transmission of sexually transmitted diseases (20%); and HIV/AIDS can cause death (20%). This is because young children have never received this information either at school or from their parents. Researchers suggest that the KOPA manager should actively cooperate with the nearest PUSKESMAS to provide information about the dangers of free sex and sexually transmitted diseases.

Abstract:

Telerehabilitation refers to the delivery of rehabilitation services via information and communication technologies. Clinically, this term encompasses a range of rehabilitation and habilitation services that include assessment, monitoring, prevention, intervention, supervision, education, consultation, and counseling. Telerehabilitation has the capacity to provide service across the lifespan and across a continuum of care. Just as the services and providers of telerehabilitation are broad, so are the points of service, which may include health care settings, clinics, homes, schools, or community-based worksites. This document was developed collaboratively by members of the Telerehabilitation SIG of the American Telemedicine Association, with input and guidance from other practitioners in the field, strategic stakeholders, and ATA staff. Its purpose is to inform and assist practitioners in providing effective and safe services that are based on client needs, current empirical evidence, and available technologies. Telerehabilitation professionals, in conjunction with professional associations and other organizations are encouraged to use this document as a template for developing discipline-specific standards, guidelines, and practice requirements.

Sprouse, Chelsea, Morgan Grant, Jaime Remines, Pramit Nadpara, and Jean-Venable Goode. "Impact of Adherence Education and Monitoring on Community Pharmacy Performance Scores and Patient Satisfaction." INNOVATIONS in pharmacy 11, no.3 (September22, 2020): 19. http://dx.doi.org/10.24926/iip.v11i3.3369.

Full text

Abstract:

Background: Adherence, specifically to noninsulin diabetes medications, statins, and renin-angiotensin system antagonists (i.e. angiotensin-converting-enzyme inhibitors (ACEi), angiotensin II receptor blockers (ARBs), and aliskiren), is a measure tracked by the Centers for Medicare and Medicaid Services (CMS) to give Medicare Part D plans a star rating; pharmacies are impacted by these star ratings. The pharmacy is given a performance score based on the measure. Some pharmacies use a performance information management platform (PIMP) that allows pharmacies to better understand performance information to impact patient care. Objectives: (1) To evaluate if a monthly adherence monitoring and education service impacts the percentage of patients adherent determined by pharmacy performance scores; (2) To determine patient satisfaction with the service in a large community pharmacy chain. Methods: A six-month prospective interventional pilot study including patients with a proportion of days covered (PDC) of less than 80% for oral diabetes or renin-angiotensin system antagonists (RASA) medications was conducted in two pharmacies of a large community pharmacy chain in Southwest Virginia. Using pharmacy internal data analytics and PIMP data, the percentage of patients who are adherent to oral diabetes or RASA medications was determined including the baseline PDC for each patient. At the start of the study, the standard of care in this large community chain pharmacy was to address adherence, follow-up in one month and every three months thereafter. In this study, pharmacists provided monthly telephonic adherence monitoring and education for a six-month period. Each session was scripted for medication adherence history, education and data collection. The pharmacist provided guidance and counseling based on how the patient answered the questions. Pharmacists gathered information about adherence patterns and behaviors using a 14 item Likert-scale and multiple choice-based questionnaire during the first session. After completion of the final adherence monitoring and education session, pharmacists gathered patient satisfaction information using an eight item Likert-scale questionnaire. At the end of the six-month period, using PIMP data, the percentage of patients adherent to oral diabetes or RASA medications was determined based on pharmacy performance scores. The data was analyzed using uni-variate and bi-variate statistics to determine if there was a difference in pharmacy performance scores from the pre-study analysis. The adherence patterns and behaviors, as well as patient satisfaction with the program was evaluated to determine factors influencing nonadherence. Results: A total of 55 patients were identified in two pharmacies, ten of which were excluded or declined involvement, leaving 45 enrolled in the adherence monitoring and education service. Of the 45 enrolled patients, ten completed the adherence patterns and behaviors questionnaire. About half of the patients were men (50.95%) with an average age of 71.17 years and taking an average of 6.55 prescription medications. All patients had Medicare Part D insurance and majority had a yearly income of less than $40,000. The average baseline PDC was 68.92. In pharmacy 1, the average performance score for oral diabetes medications trended down and the average performance score for hypertension medications trended up over the study period. In pharmacy 2, the average performance score for oral diabetes medications trended up and the average performance score in hypertension medications trended up over the study period. The adherence patterns and behaviors questionnaire revealed the majority of patients rarely forgot to take medications or run out of medications. Additionally, cost of medications did not seem to impact adherence and majority of patients knew the names and indications of their medications. Only one patient completed the patient satisfaction survey. Conclusions: An adherence monitoring and education service had mixed results in improving patient adherence and pharmacy performance scores. Only one patient completed the satisfaction survey, thus no conclusions can be made regarding patient satisfaction of the program. More research needs to be done regarding telephonic adherence programs. Article type: Original Research

Abstract:

Taiwan offers affordable access to health care through the National Health Insurance (NHI) Program, but this increases the risk of waste of medications and pollution caused by improper disposal. This research aimed to: (1) understand methods of disposal of unused medications, and public awareness of related issues, (2) understand the sources and types of unused medications and reasons for their disposal, and (3) propose improvements to current disposal practices. Data on practices and perspectives on the disposal of medications were collected from members of the public using a questionnaire, and subjected to chi-square analysis of demographic variables. The survey found that many respondents disposed of medications inappropriately, but most believed the government should establish a return system. The majority of discarded medications came from hospitals, and were most frequently discarded because patients had forgotten to take them. Based on these findings, the study proposes that receiving stations be established across Taiwan for the safe and environmentally friendly disposal of unused medications, along with a system of monthly checks and compensation for pharmacists’ costs. The study also suggests that pharmacists strengthen medication guidance, spend more time on medication counseling, better understand the reasons for medication waste, and offer health education and advice to physicians and the public.

Abstract:

105 Background: Oncologists often embrace the concept of having discussions with patients regarding end-of-life care. Still, challenges remain with initiating these discussions early in the metastatic setting so that the full ACP process can be satisfied. A framework and infrastructure are needed to systematically initiate and promote the ACP process at a time when metastatic disease is identified and patients are better able to make thoughtful decisions regarding their future care. Methodology is needed for documenting and updating the ACP process within the patient’s health record. Methods: During a 15 month period, 32 independent sites of service within a nationwide community oncology network implemented an electronic health record (EHR)-based program for initiating ACP conversations with patients having a metastatic cancer diagnosis. Patients were identified for program engagement based on clinical criteria populated within the EHR. Identified patients received a brief introduction to ACP by the physician or other clinician and were encouraged to meet with a trained ACP Counselor to engage the full process. ACP Counselors provided guidance to the patient to discover and share personal values and goals for care. Values for care were documented within the EHR. Counselors provided appropriate patient education regarding code status and state-specific advance directives. At end-of-life, practices included hospice and death details within the EHR. Results: For metastatic patients (n = 29,389), ACP counseling was provided for 1,034 individuals (4%). Code status was visible in 3648 health records (12%) and patient values were included for 376 patients (1%). 2,206 patients (8%) transitioned to hospice care during the 14-month period. EHR-based infrastructure allowed for ongoing ACP visibility by the healthcare team. Conclusions: Use of an EHR-based ACP process demonstrates promising outcomes for realizing meaningful delivery of ACP. Essential components are patient identification early in the metastatic trajectory, assessment of patient values for care, thorough patient education regarding code status and advance directive documents, and full visibility of the ACP process within the health record.

Abstract:

ObjectivesGlobally, there is emerging evidence on the use of community health workers and volunteers in low-income and middle-income settings for the management of non-communicable diseases (NCDs), provision of out-of-clinic screening, linkage with health services, promotion of adherence, and counselling on lifestyle and dietary changes. Little guidance exists on the role of this workforce in supporting NCD care for refugees who lack access to continuous care in their host country. The goals of this work were to evaluate the current roles of community health volunteers (CHVs) in the management of diabetes and hypertension (HTN) among Syrian refugees and to suggest improvements to the current primary care model using community health strategies.Setting and participantsA participatory, multistakeholder causal loop analysis workshop with representatives from the Ministry of Health of Jordan, non-governmental organisations, United Nations agencies, CHVs and refugee patients was conducted in June 2019 in Amman, Jordan.Primary outcomeThis causal loop analysis workshop was used to collaboratively develop a causal loop diagram and CHV strategies designed to improve the health of Syrian refugees with diabetes and HTN living in Jordan.ResultsDuring the causal loop analysis workshop, participants collaboratively identified and mapped how CHVs might improve care among diagnosed patients. Possibilities identified included the following: providing psychosocial support and foundational education on their conditions, strengthening self-management of complications (eg, foot checks), and monitoring patients for adherence to medications and collection of basic health monitoring data. Elderly refugees with restricted mobility and/or uncontrolled disease were identified as a key population where CHVs could provide home-based blood glucose and blood pressure measurement and targeted health education to provide more precise monitoring.ConclusionsCHV programmes were cited as a key strategy to implement secondary prevention of morbidity and mortality among Syrian refugees, particularly those at high risk of decompensation.

Abstract:

Aims/Objectives. This study assessed the adherence to iron and folic acid supplementation and the associated factors among pregnant women attending antenatal care at Bwindi Community Hospital, in Western Uganda. Methods. This was a cross-sectional study that used an interviewer-administered questionnaire and reviewed medical records. Binary and multivariable logistic regression analyses were used to identify factors associated with iron and folic acid supplementation. Adjusted odds ratio (AOR) with 95% confidence interval (CI) and p value < 0.05 were used to assess for statistical significance. Results. We enrolled 438 pregnant women aged 16 to 41years. Participants’ mean age (±standard deviation (SD)) was 25.9 (±3.17) years. The self-reported adherence to iron and folic acid supplementation (consumed ≥4 tablets a week or 20 tablets in a month daily without missing the prescribed dosage) was 22.37% ( N = 98 ). Among the adherent pregnant women, the reported reasons (and their respective proportionality) for adherence were getting advice and counseling from the healthcare worker about the good effects of iron and folic acid supplementation ( N = 34 , 34.69%) and knowledge about the health benefits of iron and folic acid supplementation such as preventing anemia ( N = 16 , 16.33%), among others. On the other hand, the reported reasons (and their respective proportionality) for iron and folic acid nonadherence were forgetfulness ( N = 158 , 46.47%), taking too many pills ( N = 7 , 2.06%), not knowing the usefulness of iron and folic acid supplementation ( N = 29 , 8.53%), fear of the side effects of the medication ( N = 119 , 35.00%), and not getting the supplement from the hospital ( N = 27 , 7.94%). Bivariable and multivariable logistic regression analyses indicated that pregnant women who were primigravida ( adjusted odds ratio AOR = 4.5 ), who have parity of 2 or 3 ( AOR = 3.4 ), who perceived importance of iron and folic acid supplementation to prevent anemia ( AOR = 2.9 ), and who considered it important to take iron and folic acid supplementation ( AOR = 2.9 ) showed a statistically significant association with adherence to iron and folic acid supplementation. Moreover, pregnant women who perceived the risk of not taking iron and folic acid supplementation ( AOR = 5.2 ), those who received sufficient health education regarding the goals of iron and folic acid supplementation as well as the dangers of not taking the supplements ( AOR = 4.4 ) and adequate counseling, and those who obtained an explanation of the effects of iron and folic acid ( AOR = 4.8 ) showed a significant association with adherence to iron and folic acid supplementation. Conclusion. This study found a low adherence of iron and folic acid supplementation and was associated with obstetric and client- and health system-related characteristics. To this end, there is a need for individualized strategies targeting such factors and intensifying health education, guidance, and counseling to optimize adherence to iron and folic acid supplementation.

Abstract:

We’ve all seen the numbers—In 2016, the Institute of International Education (2020) reported a 3% decrease in first-time enrollment of international students in the United States, amounting to nearly 10,000 students. This initial dip, a first since the Institute for International Education began collecting data in 2005, has continued in both 2017 and 2018 (see Table 1). Some in the field have attributed the trend to an increase in the price of education, heightened global competitiveness, and a decrease in sponsored scholarships from key markets. However, many have also pointed to the U.S. political climate following the election of President Donald Trump (Glum, 2017; Rose-Redwood & Rose-Redwood, 2017; Saul, 2018; Smith, 2017). While some international educators have called the impact of Trump immigration policies on international college students trivial, other institutions have noted concerns over the experiences of international students (Deruy, 2017; Pottie-Sherman, 2018). In response to the 2016 election, a number of institutions quickly reacted with messages of support to their international community. Universities across the nation leapt to action. Most notably, the #YouAreWelcomeHere campaign swept the nation, with more than 380 institutions of higher education participating (NAFSA, 2020). With its “America First'” vision, the Trump administration has focused its efforts in three main areas of immigration: border security, interior enforcement, and employment (The White House, 2018). The President has encountered setbacks in achieving some of his goals, but many proposed changes in immigration law have succeeded (Pierce, 2019). Among his most recent actions are presidential proclamations that suspend certain employment-based visas to preserve domestic jobs and support the U.S. economic recovery amid the Coronavirus pandemic. In order to pursue its aggressive immigration agenda, the administration has crafted and adopted a powerful narrative for the American public that has been distributed through digital and news media. It can be argued that this narrative, employed as a tool for supporting policy change, has had an impact on the feelings of safety, security, and belongingness for many people, including international students and scholars in the United States. The Narrative Policy Framework (Shanahan et al., 2018) argues that stories are integral in shaping the entire policy cycle, from agenda setting through implementation. It acknowledges a narrative’s four main structural elements: setting, characters, plot, and the moral of the story. Beyond its structure, the Narrative Policy Framework also suggests that policy actors may utilize strategies to move their audiences in one direction or another, serving as a powerful catalyst for change (Shanahan et al., 2018). Burgeoning literature in the field of international student services hints at this connection between an increasingly unwelcoming environment for international students and their growing feelings of insecurity (Bartram, 2018; Mathies & Weimer, 2018; Rose-Redwood & Rose-Redwood, 2017). International student support offices across the world may vary in organizational structure and the range of services they provide, but all share the responsibility of assisting international students in their educational and cultural transition to campus (Ammigan & Perez-Encinas, 2018; Briggs & Ammigan, 2017). As international educators, it is our responsibility to reach across disciplines for tools that help us better understand and serve our communities. While some of us have institutional responsibilities to administer regulatory compliance with shifting immigration policies and procedures, we must also acknowledge that the support model for our students may need to be recalibrated so we can directly address the potential impact of other environmental factors, including political narratives. Below, we offer a few recommendations for administrators and support staff to consider as they bolster support for their international community. Incidentally, these propositions might also be relevant to many non-U.S. institutions that are addressing similar situations and issues on their respective campuses internationally. Provide access to accurate immigration advising. Amid confusion and varying perspectives on changing immigration policies, it is important for designated university officials to remain accessible to students and scholars who seek timely and factual guidance on their visa status and employment options as per official government regulations. Establish an open forum for addressing concerns. Some students, despite struggling to understand the effects of a changing political climate, may experience social withdrawal and hesitate to come forward. Institutions must consider creating a safe and supportive space for dialogue. This also includes regularly assessing the needs and challenges of their students. Partner with service offices, academic units, and student organizations on campus to develop collaborative resources that can help address the overwhelming fears and anxieties among international students and scholars, and ensure their wellbeing and academic success. Develop initiatives with local government and community organizations to create a welcoming home and friendly setting for international visitors. International student support offices can play a leadership role in developing supportive networks and connections with the wider community. Implement culturally sensitive orientation programs and early interventions that support international students during times of high stress to help them with their academic, social, and cultural adjustment to campus. As university administrators and staff recognize the impact of political narratives on the wellbeing of our international communities, it is critical that we remain proactive in providing support services that are intentional and inclusive in nature. Such initiatives not only enhance the student experience but can help advance diversity and internationalization efforts across the institution.

Abstract:

IntroductionPhysical Medicine and Rehabilitation is a goal oriented and patient centered specialty which focuses on functional restoration and quality of life of persons with disability. The patterns of injuries among survivors of recent disasters have, range from mild (single limb fracture) to catastrophic (spinal cord injury, amputation, traumatic brain injury). Historically physiatrists have not participated the acute disaster management phase or in the emergent post disaster rehabilitation planning. This task is usually relegated to the trauma, orthopedic and general surgeons.MethodologyAuthors had firsthand experience in the acute and emergent care and rehabilitation of trauma patients after Pakistan, China and Haiti earthquakes. An electronic literature search (English, 1965–2010, Key words: trauma, rehabilitation, disability, spinal cord injury, amputation, disaster, nerve injury) was carried out. Experience sharing through committees, online forum, and communications were conducted with physiatry colleagues internationally.ResultsIn these three recent earthquakes, Physiatrists provided direct patient care, including guidance in the evacuation of survivors with pre-existing disabilities, transport of persons with spinal trauma, treatment of wounds, fractures, pain, spinal trauma patients and persons with amputations. Physiatrists devised appropriate plans for conservative management of fractures. Education of local staff and coordination of rehabilitation was initiated. Monitoring, prevention and treatment of secondary complications including prolonged immobility, pressure ulcers, chronic pain, urinary, bowel and respiratory dysfunction was performed. Physiatrists helped in patient counseling and family education.ConclusionPhysiatrists by virtue of their training and skills are in a better position to manage the disabilities, including direction of rehabilitation and community integration, prevention of complications, and education and training of health workers and teams. Timely rehabilitation interventions for Spinal cord injuries and lower limb amputations following the Pakistan, China and Haiti earthquakes resulted in reduction in morbidity and mortality among those with catastrophic injuries.
